Upon acceptance as a member of the London Cannonballs Boccia Club, the applicant agrees to abide by the rules and procedures of the Club, as approved through rules and by-laws. The applicant should demonstrate good sportsmanship with fellow members and other athletes. The applicant is expected to help with fundraising events. It is understood and agreed that the Club and/or any of its officials, affiliates or volunteers does not assume responsibility for any injury, damage or loss resulting from any accident from known or unknown conditions howsoever caused.
I authorize the use of my image and/or name in any written, radio, television, or other media coverage of the London Cannonballs Boccia Club.
